This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community.
The healthcare teams at St.
Paul's PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a full-time position in our Social Work Department.
Job
Summary:
Under the direction and supervision of the Social Work Supervisor, the Support Specialist assists to skillfully interact with participants with challenging behaviors by re-directing, de-escalating, and assisting participants in using positive coping skills.
Coordinates with the IDT regarding participants' cognitive impairment/ mental health and or behavioral issues.
Qualifications & Requirements:
Education:
Associates or Bachelors degree in mental health or related field strongly preferred.
High school diploma or equivalent required.
Valid California Driver's License.
Experience:
A minimum of six months experience with mental health and one year experience in working with the frail or elderly population required.
Shall have either training or related experience in the job assigned.
Bilingual in Spanish preferred Skills and Knowledge:
Ability to demonstrate emergency response skills.
Ability to demonstrate good decision making in dealing with safety issues and with aggressive and/or intoxicated clients, if required Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
Basic knowledge and excellent communication techniques in working with individuals with dementia Skilled in identifying and recommending problem resolution.
Ability to work effectively with culturally, economically, and educationally, diverse populations.
Strong clerical skills with computer literacy St.
